Discharge from hospital can happen quickly. If you live in Etobicoke or elsewhere in Toronto, starting to plan early with the care team helps avoid surprises at home. Medical guidance always comes from the clinical team.
Ask the discharge team
- What is the expected discharge date, and what written instructions will we receive?
- Have publicly funded home and community care services been requested or arranged, and who is the contact?
- Which follow-up appointments, referrals, or equipment are needed?
- Which daily activities need help, and which are for healthcare professionals only?
Prepare the home
- Confirm transportation and who will be there on arrival, including stairs or building elevator access.
- Clear walkways, keep everyday items within reach, and plan easy meals.
- Keep discharge papers and the care team’s contact details together.
Non-medical support at home
ApexHealth can discuss non-medical support in Etobicoke, Toronto, and the GTA: PSW help with agreed daily routines, companionship, meals, light housekeeping, caregiver respite, and overnight support, depending on needs and availability. We do not provide nursing, medication management, or medical care.
If symptoms are urgent or worsening, follow the care team’s instructions or contact emergency services.
